Doripenem is a new carbapenem that has a broad antibacterial spectrum against Gram-negative bacteria and aerobic Gram-positive bacteria. We also explored the relationship between pharmacokinetic parameters and clinical outcomes. We prospectively studied 55 adult patients with severe infection receiving doripenem in addition to other antimicrobial agents, on three different dosing regimens. Serial blood and urine samples were collected and the concentrations of doripenem were measured using an HPLC method. The population pharmacokinetics of doripenem was investigated using nonlinear mixed-effects modelling. A Monte Carlo simulation was conducted to estimate the probability of target attainment (PTA) for various doripenem doses. The median age of the patients was 59.5 years and there were 33 male and 22 female patients. There were 32 patients with sepsis, 13 patients with severe sepsis and 10 patients with septic shock. The population pharmacokinetic analysis showed that clearance (CL) and volume of distribution (Vd) of doripenem were low in critically ill patients with severe infection.
